Waiver Forms

All skaters must have a completed and signed waiver on file prior to participating.
If you are bringing multiple minors, please use the Waiver with Multiple Minors form.

Please download, complete, and bring the signed waiver with you to your party or open skate.

• Standard Waiver – for individuals & single minors
   
• Multi-Minor Waiver – for parents/guardians signing for more than one child
